Hypothesis / aims of study
Stress urinary incontinence (SUI) is the involuntary loss of urine with activities causing increased intra-abdominal pressure. Prevalence varies from 18% in India to 26.4% in USA. Diagnosis is made by symptoms assessment including frequency volume chart and assessing quality of life questionnaire, abdominal and pelvic examination, cough test, voluntary pelvic floor muscle contraction and post void residual urine. International Consultation on Incontinence Questionnaire (ICIQ) calculates the score by asking the patient how often she leaks, how much she leaks and on a scale on 0-10 the impact on her quality of life.
Thus, the study access the short term results of autologous rectus fascia pubovaginal sling surgery in treatment of female stress urinary incontinence.
Study design, materials and methods
It was a prospective study on 27 women between 25-65 years of age with urodynamic proven SUI who underwent autologous rectus fascia pubovaginal sling surgery. Preoperative and postoperative ICIQ- SF (International Consultation on Incontinence Questionnaire short form) score and urodynamic studies were done in all women.
Results
Age ranged from 33 to 58 years with mean being 44.8± 5.9 years, arity ranged from 1 to 6 with mean being 3.12 while BMI ranged from 20.2± 35.18 Kg/m2 with mean being 27.12 ± 4.1 Kg/m2. A total of 11 patients (40.7%) were menopausal. Demonstrable SUI on cough test was seen in all 27 (100%) patients and Bonney’s test was also positive in all 27 (100%) patients. Pre- operative ICIQ-SF score ranged from 13 to 20 with mean being 16.8±2.12 while pre- operative Pdet at Qmax (cmH20) ranged from 4.8-38.5 with mean being 25.18±8.18. Operative time ranged from 45 to 85 minutes with mean being 58.5±6.8 minutes. Blood loss ranged from 100 to 600 ml with mean being 120 ml. Pots operative hospital stay ranged from 40 to 12 days with mean being 7.2±1.8 days. Mean duration of cathetarization ranged from 3 to 21 days with mean being 4.8± 2.8 days. Complete cure rate was seen in 25 (92.5%) cases while partial cure rate was seen in 2 (7.4%) cases. Mean ICIQ score was zero after the surgery. Post- operative urodynamic Pdet at Q max (cm H20) ranged from 15.2 to 35.4 with man being 29.8± 5.9. Complications observed were urinary retension needing >72 hours of cathetarisation in 8(29.6%) women, urgency in 3 (11.1%), UTI in 2 (7.4%), wound hematoma in 2 (7.4%), surgical site infection in 5(18.5%).
Interpretation of results
Autologous rectus fascia sling showed complete cure rate in 25 out of total 27 patients.
